Training Agreement
I give Fairy Tail K9 LLC and its affiliates permission to train my dog(s). I understand that Fairy Tail K9 LLC does implement the use of training tools, and I give Fairy Tail K9 LLC permission to use said tools on my dog(s). I understand that training tools may include, but are not limited to: martingale collars, slip leashes, prong collars, and electronic collars. I understand that I will continue to use any and all training equipment properly and appropriately upon my dog(s) completion of the aforementioned training program. I give Fairy Tail K9 LLC permission to take my dog into public spaces and understand the risks associated. I release Fairy Tail K9 LLC from any harm or accident that may come to my dog during the above-stated training program.
By purchasing, scheduling, and/or participating in training, you represent that you are the legal owner of the named dog(s) and/or have permission to purchase and/or participate in training programs through Fairy Tail K9 LLC. You understand that participation in dog training programs may result in accident, illness, injury, death, or loss/escape of the participating dog(s). You understand and are aware of these risks and assume all responsibility. You understand that you are liable for any damage, injury, accident, and/or death to the above-mentioned dog(s), caused by the aforementioned dog(s), and/or to any other dogs or individuals. By agreeing to these terms you agree to release Fairy Tail K9 LLC and its affiliates from any and all claims, losses, actions, suits, damages, and demands of any kind.
By purchasing, scheduling, and/or participating in training you understand that Fairy Tail K9 LLC does implement the use of training tools including, but not limited to: slip leashes, training collars, electronic collars, and more. I understand that all training tools will be used properly and appropriately during the selected training program, and I will continue to use all equipment properly and appropriately upon the completion of the aforementioned training program.
I understand that there are many factors out of Fairy Tail K9 LLC’s control such as, but not limited to: other dogs, motor vehicles, hot pavement, and more, and release Fairy Tail K9 LLC of any liability associated. I understand that my dog(s) will be transported by Fairy Tail K9 LLC by motor vehicle and understand the risks associated, and hereby release Fairy Tail K9 LLC of any liability. I give permission for Fairy Tail K9 LLC and its affiliates to change my dog(s) food (quantity, brand, schedule, etc) as they see fit for the benefit of my dog. I give Fairy Tail K9 LLC permission to seek veterinary care for my dog(s) on my behalf, as seen fit for the benefit and wellbeing of my dog(s). I give Fairy Tail K9 LLC permission to groom and care for my dog during the course of its training program and assume responsibility for any costs and fees associated.
This contract is validated by the signatures below in total and as approval for future services without additional written authorization.
