Fairy Tail K9 LLC will endeavor to create as safe an environment as possible for the training of my dog and will offer only sound, safe, and responsible training and training instructions. However, to the extent that Fairy Tail K9 LLC is insured for any unintentional or negligent errors, omissions, or incorrect assertions, Fairy Tail K9 LLC will be responsible for any such acts or omissions, but only to the extent of such insurance. I have been told by Fairy Tail K9 LLC and understand the inherent risks of owning a dog, including but not limited to the risk of dog bites to myself or others, and consequently, I am and will remain responsible for the actions of my dog at all times, and I hereby agree to indemnify and hold harmless Fairy Tail K9 LLC of any and all claims of injury, expense, costs, or damages caused by my dog. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service.
I authorize Fairy Tail K9 LLC to take my dog into public spaces during his/her stay for the purpose of training my dog.
Payment Policy: I understand that all payments made are NON-REFUNDABLE regardless of my decision to complete any requested training program. If participating in a payments plan, I understand that I am responsible for making any and all payments on time. I also understand that I am responsible for contacting Fairy Tail K9 LLC for any changes to forms of payment. I understand that payment is due in full prior to the end of each training program.
Cancellation Policy: I understand that any cancellations or reschedules must be made AT LEAST 24 hours in advance. I understand that it is my responsibility to contact a representative at Fairy Tail K9 LLC to notify of any cancellations or rescheduling. I understand that if I am more than 20 minutes late to an appointment with Fairy Tail K9 LLC that is considered a no-show. I understand that a no-show will result in forfeiting my lesson and/or service, no re-scheduling will be available to me, and no reimbursement will be given.
Grooming Policy: I give Fairy Tail K9 LLC permission to groom and care for my dog(s) as needed for its health and well-being during their stay. Care includes, but is not limited to, bathing, nail trims, trimming and/or cutting of hair, etc. I understand that I am responsible for any costs and fees associated with grooming.
Abandonment Policy: I understand that I am expected to pick up my dog immediately upon completion of any training service(s). If I fail to pick up my dog upon its completion date there will be a $50 per day charge for any additional days/boarding. I understand that all fees and payments must be completed before the release of my dog into my custody. I understand that if I fail to pick up my dog on time and do not contact Fairy Tail K9 LLC to make arrangements regarding pickup, I have 7 days to pick up my dog. If I do not pick up my dog within 7 days of the initial pick-up date, my dog will be considered abandoned and Fairy Tail K9 LLC will assume full custody of the dog.
