Waiver and Liability Agreement must be completed and agreed to in order for furbaby to enter our facility for any/all services!

I (the “Owner”) hereby represent that I am the legal owner of the dog described above
(the “Dog”) to be enrolled in Faith's Furever Pet Needs And Resort.

I, the undersigned, am satisfied that Faith’s Furever Pet Needs And Resort facility is
maintained for the health, comfort, and protection of my animal(s).

I understand and agree that in admitting my dog(s), Faith’s Furever Pet Needs And Resort has
relied upon my representation that my dog(s) is/are in good health and have not
harmed or shown aggressive or threatening behavior towards any person or any
other dog.

I hereby release Faith’s Furever Pet Needs And Resort, its directors, officers,
shareholders, employees, assistants, members and agents from any and all
liability, claims, suits, actions, loss, injury or damage of any nature or
kind, which I, the Dog and any third party or their pet may suffer, including specifically,
but not without limitation, any accidents, injury, damage, loss or death of my animal at Public Off-Leash Parks/Pathways, on or off the premises at 80 Millennium
Drive, Suite 204, Quispamsis, NB E2E 4B1 whatsoever while in attendance and participation at Faith's Furever Pet Needs And Resort.

I allow Faith's Furever Pet Needs And Resort staff to contact my/a veterinarian as deemed
necessary should any injuries require medical attention. I agree that I am responsible for any medical and legal responsibility of my pets' actions.

I approve Veterinary care for my dog up to the amount of $500, if Faith’s Furever Pet Needs And Resort cannot reach the owner prior to receiving veterinary care. If care exceeds the amount of $500, veterinary care will be postponed until the
owner is contacted.

 I represent that my dog is in good health, is current on all required vaccinations (DHLPP,
Bordetella and Rabies), is current and has not been ill with any known contagious diseases. A copy of all medical records will be submitted to Faith's Furever Pet Needs And Resort on the first day of admittance.

I understand that Flea/Tick medication is recommended but not mandatory and understand that these may be contagious in pet facilities that house multiple pets.

I understand that while my dog is fully vaccinated that vaccines are not 100% fool proof and there is still a risk (albeit minimal) that my dog may contract a contagious virus/disease.

I agree that should this occur. I agree to allow 7 days waiting period after my dog has
had his/her vaccinations to allow the vaccines to reach full protection
potential and to ensure my dog has not had any negative reaction to the vaccines. Should I allow my dog to stay at Faith’s Furever Pet Needs And Resort
in a shorter than 7 days wait period from day of vaccinations, I understand that my dog could be at risk of contracting a contagious virus/disease.

I agree to disclose any previous or current medical issues/concerns of my dog so that Faith’s Furever Pet Needs And Resort staff can decide of suitability for asked service.

I understand that I must pay in full for care and services rendered before the Dog is released from Faith's Furever Pet Needs And Resort into my custody.

I understand that I am to walk the Dog in and out of the facility on a leash, and in a safe and expedient manner.

I agree that Faith’s Furever Pet Needs And Resort is not responsible for any lost or
stolen items that accompany my dog during daycare. Ie. Jackets, booties, collars and leashes.

I agree that the Dog may be videotaped, photographed, and/or recorded. Faith's Furever
Pet Needs And Resort shall be the exclusive owner to the results and all proceeds of such taping, photography, and recording.

I certify that I have read and understand the rules and regulations herein and that I
have read and understand this agreement.

Non Daycare only - Faith's Furever Pet Needs hours are 7:00 am to 6:00 pm (“Closing Time”) Monday to
Friday and 10:00 am to 5:00pm Saturday. I agree that management retains the right to amend the hours of operation at any time.

Dog Daycare Service Only - I understand that although all dogs are fully supervised that incidents of injuries may occur. This includes that my dog
could receive bites, scrapes and scratches from his/her playmates.

I understand I am solely responsible for any harm caused by the Dog to another dog while they are at Faith's Furever Pet Needs And Resort. If for any reason Faith’s Furever Pet Needs And Resort is unable to further care for the Dog, I must have an emergency contact available to pick up and care for the Dog.

I represent that my dog will be evaluated and deemed suitable for daycare.

I understand that female intact dogs can only attend daycare up to the age of 6 months or is spayed, which ever one comes first.

I understand that if my male dog is over the age of 6 months and is intact and is exhibiting Intact aggressive behaviours toward other male dogs, Faith’s Furever
Pet Needs And Resort will contact me for immediate pick up.

I understand that my dog must be licensed by the Town of Quispamsis. Faith’s Furever Pet Needs And Resort will not be responsible for bylaw infraction
tickets during walks to off-leash or on path walks. The daycare hours at Faith's Furever Pet Needs And Resort are 7:00 am to 5:30 pm (“Closing Time”) Monday to
Friday.

I agree that management retains the right to amend the hours of operation at any time.

I agree to pick up the Dog prior to Closing Time. It is hereby acknowledged that should I
fail to pick up the Dog prior to closing a late charge fee of $25 may be applied. Faith’s Furever Pet Needs And Resort will inform client of late pick up to remind of the late fees.

I understand that cancelling a reservation within 24 hours may be subject to a cancellation fee of $30+tax and added to your account if re-occurring.

I understand that to maintain control and exhibit leadership in a large group of un-kenneled/kenneled dogs certain humane training and disciplinary methods may be used as the owners or staff of Faith’s Furever Pet Needs And Resort sees fit given
the circumstances.

I agree to abide by the rules and regulations and accept all terms, conditions, and statements of this agreement and confirm the truthfulness of the contents of
the application form completed by me.

I am of the full age of eighteen years of age.

By filling out the assessment form, you fully consent to this waiver and agreement in its entirety.
