1. Acknowledgment of Risk
I understand that in-home pet care and entering a petā€™s home environment involve inherent risks, including but not limited to:

  • Bites, scratches, or other injuries caused by pets during in-home visits Ā 
  • Ā Illness or unexpected health issues Ā 
  • Property damage or accidental loss Ā 

    I accept these risks and release Five Happy Tails and its owners from any liability for injury, illness, or loss that may occur during drop-in visits.


2. Pet Behavior & Safety
I certify that my pet(s):

  • Have no history of unprovoked aggression toward people, or I have fully disclosed any behavioral concerns Ā 
  • Have disclosed any relevant behaviors (anxiety, fear, territorial tendencies, escape risk, etc.) Ā 
  • Can be safely cared for in a home environment Ā 

    I understand that if a safety risk is present, the visit may be ended, and I remain responsible for payment.


3. Medical Care AuthorizationĀ 
I authorize Five Happy Tails to seek veterinary care in case of illness or injury. I understand that:
Ā 
  • I am responsible for all veterinary costs Ā 
  • Reasonable efforts will be made to contact me prior to treatment Ā 
  • In emergencies, care may be sought without prior approval Ā 


4. Vaccinations & Preventative Care
I confirm that my pet(s):

  • Are up to date on required vaccinations (as applicable) Ā 
  • Are free of fleas, ticks, and contagious illnesses Ā 


5. Home Access & Security
I agree to provide safe and reliable access to my home (key, lockbox, code, etc.).

I understand that:

  • Entry instructions must be accurate and functional Ā 
  • If entry cannot be gained, the visit may be missed and still charged Ā 
  • Five Happy Tails is not responsible for issues related to home access failures or security systems Ā 


6. Property & Home Responsibility
I understand that Five Happy Tails is not responsible for:

  • Damage to my home or belongings caused by my pet(s)Ā 
  • Accidents that occur between scheduled visits Ā 
  • Issues resulting from unsecured areas, faulty equipment, or unsafe conditions in the home Ā 


7. Rules & Policies
I have read and agree to abide by Five Happy Tailsā€™s policies, including:

  • Visits are approximately 30 minutesĀ 
  • Visits occur within a scheduled time window and are not guaranteed at an exact time Ā 
  • Care will be provided based on the instructions given Ā 
  • Full payment and cancellation policies Ā
