Thank you for your interest in the Flying Paws Animal Assisted Therapy Dog pathway. Therapy work requires careful assessment, emotional stability, and a strong human–dog partnership, so we intentionally review each application to ensure the process supports both dog welfare and community wellbeing.


This questionnaire helps us understand your dog’s temperament, history, training experience, and suitability for therapy environments before moving forward. Completing this form thoroughly allows us to guide you with clarity, honesty, and realistic expectations around the therapy journey.

Next Steps After Submission

Once your application has been reviewed, you will receive an email outlining whether an initial assessment is the appropriate next step.

Submitting this form does not guarantee therapy program placement. All dogs must meet behavioural, emotional, and welfare suitability criteria before progressing.

Welfare & Facility Requirements

For the safety and wellbeing of all dogs, handlers, and participants, dogs attending any Flying Paws sessions must be up to date with vaccinations or hold a valid titre test completed within the last 12 months.

Flying Paws reserves the right to recommend alternative training pathways if therapy work is not currently in the best interest of the dog or handler.
