Thank you for applying for Puppy Private Sessions with Flying Paws. We accept a limited number of private puppy clients to ensure each family receives structured, hands-on support during these important early stages.

This questionnaire helps us understand your puppy’s routine, development, environment, and your goals so we can prepare intentionally before your first session. Early preparation allows us to create personalised guidance that builds confidence, emotional wellbeing, and real-life skills — not quick fixes.

If you have selected other or have a specific time within the times above please list your availability in as much specific detail as you can. We may be able to fit you in outside of these times around our class schedules. Please note, Sundays and Mondays are not available and Saturday are unavailable due to our class times, but Saturdat evening sessions may available in extreme circumstances. We are currently booking 3-4 weeks in advance. *

Next Steps After Submission

Once this intake form has been completed, you will receive an email containing a link to select a time within your chosen Puppy Private package.
Selecting your session time will automatically trigger a separate confirmation email with deposit details required to secure your booking.

Submitting this form does not guarantee immediate availability, as we accept a limited number of private clients each month to maintain the calm, structured experience Flying Paws is known for.

Facility & Vaccination Requirements

For the safety and wellbeing of all dogs attending our facility, puppies must be up to date with their vaccinations.

Proof of current vaccinations, or a valid titre test completed within the last 12 months, must be provided prior to attending any in-person sessions.
