DOG OWNER REPRESENTATIONS; DUTY TO DISCLOSE.ย Dog Owner represents and warrants to McWreath Dog Training (the Company) that:ย

a.ย The dog is (and will be during his/her stay) current on all required vaccinations listed below, and such other vaccinations as is required by the Company. ย
- Bordatella
- Rabies

Leptospirosis (sometimes part of the DHPP vaccine above, shown as "DH'L'PP)
b.ย All required vaccines will be given no less than one (1) week prior to entry at the Company facilities.ย
c.ย Dog Owner agrees to provide documents confirming such vaccinations as requested by the Company, and shall provide proof of current vaccinations prior to any entry at the Company facilities, or any evaluation made by the Company:

d.ย The dog is (and will be during his/her stay) current on Flea & Tick Prevention and is free of fleas, ticks, worms, and other insects and parasites.
