I understand that new dogs may be accepted up to 12 months of age that have not been spayed/neutered. Dogs that begin to demonstrate signs of disruptive behaviour in the group such as excessively mounting others, going into heat, show aggressive behaviour towards other dogs, or excessively mark the site to the point that it becomes too difficult to keep clean will be required to pause daycare at the discretion of Mindful Mutts as early as 6 months old due to challenges that directly impact our philosophy of "low stress" and "easy management" for the sake of mental & physical health in both YOUR dog, and the others in the group. Packages may be paused for up to one year, and a retrial day with a fee of $45 or 1 daycare credit from an existing package will be required to start back up once your dog is spayed or neutered. The Owner understands that Mindful Mutts does not allow vasectomies or ovary-sparing spays in place of spaying or neutering. We may request proof of sterilization, please have it handy just in case! If you do not plan on having your adult dog spayed or neutered, we may not be the best fit and it may be a good idea to look at other service providers who may better suit you and your dog's needs.

The Owner certifies that all required vaccinations for the Pet(s) (Bordetella, Distemper, Parvovirus, Parainfluenza) are up to date, or that associated titer tests have been done, and is aware and acknowledges that vaccines do not protect pets against all communicable diseases. The Owner acknowledges that Mindful Mutts may require all pets in their care to be re-vaccinated or to be vaccinated for a disease that may be considered “new” or “dangerous” and agree to update vaccinations within 48 hrs of written request from Mindful Mutts, and provide documents with proof of vaccine upon subsequent visit to Mindful Mutts.* *If titre test was done, please provide a copy of the documentation to Mindful Mutts with this form or simply attach your pet's proof of most recent vaccinations.

In an emergency, Mindful Mutts will try to contact The Owner or emergency contact and personal veterinarian as provided by the Owner. However, the Owner understands and agrees that in an emergency there might not be the time to do so before the administration of care. Therefore, in the event of a sudden illness or injury, The Owner grants Mindful Mutts full authority to make decisions involving medical treatment of the Pet(s) and authorizes Mindful Mutts to take the Pet(s) to the veterinarian at Reimbursement will be required within 14 calendar days of billing, via etransfer to info@mindfulmutts.ca Please specify the amount you authorize Mindful Mutts to spend on The Owner's behalf in case of emergency health requirements, by doing so you agree to these terms:

The Owner understands that the Pet(s) will interact with other pets while at Mindful Mutts. The Owner understands and agrees that the Pet(s) may sometimes receive minor cuts and scratches while playing and socializing, and the Owner may not be notified of any cuts and scratches until the Pet(s) is/are picked up and potentially even via email or text after the fact. Mindful Mutts agrees to exercise precaution and reasonable care of the Pet(s) while at Mindful Mutts. The Owner acknowledges and agrees that they are assuming all risk of illness, disease, harm or otherwise to the Pet(s) when allowing the Pet(s) to stay at Mindful Mutts, and agrees to hold Mindful Mutts harmless in the event of an injury or accident. The Owner understands the risks associated with being around and interacting with any and all dogs. The Owner acknowledges and agrees that they are assuming all risk of illness, disease, harm or otherwise to themselves, persons and pets when at Mindful Mutts, and agrees to hold Mindful Mutts harmless in the event of an injury or accident. The Owner acknowledges that they are solely responsible for the Pet(s) behaviour while it is at Mindful Mutts, and agrees to incur any costs associated with injuries to another person or pet, or damages to facilities. The Owner agrees to inform Mindful Mutts of any and all pre-existing conditions that the Pet(s) may have (e.g., illness, aggression), and to reimburse Mindful Mutts for any losses or damages suffered as a result of failing to inform Mindful Mutts of any pre-existing conditions.

The Owner agrees that Mindful Mutts is a pet care service, that follows the guidelines of Dr. Sophia Yin's Low Stress Handling which Mindful Mutts is certified in. The Owner also understands that all punitive training tools and techniques, will NOT be welcomed or used by our staff or by the Owner on any Pet(s) within the facility or care of Mindful Mutts at any time. The Owner understands we do not allow or condone the use of any dominance theory or Cesar Milan “dog-whispering” including but not limited to; leash correction, alpha rolling, punishment or corrections of ANY kind. The Owner understands that Mindful Mutts reserves the right to not allow choke chains, prong collars , “e-collars” shock collars, or any other training tools that do not fall under the categories deemed by Mindful Mutts, as “fear-free” “harm-free” “Low stress” “Positive reinforcement” and agree to be charged for alternative training and management tools approved by Mindful Mutts, or for the Pet(s) to be turned away at any time. If The Owner chooses not to comply with these standards, they are forfeiting any costs paid for any and all services offered by Mindful Mutts. The Owner understands that we will not allow the use of force, fear, or intimidation, or the use of choke chains, prong collars, shock collars, or any tools Mindful Mutts deems adversive under any circumstance. If it is discovered that these tools are used outside of daycare, the owner and pet(s) will be asked not to return, their package will be forfeited with no refund. Mindful Mutts reserve the right to contact former or current trainers, dog walkers, behaviourists and any other dog care provider, about any information about pet(s) history, training or current behaviours. Mindful Mutts reserves the right to request that the Pet(s) are not attending another dog daycare, if we feel it is not beneficial, and in fact hindering their success at Mindful Mutts although it does not mean they are not able to do so unless it becomes problematic. By completing this form, you agree to these terms.

The Owners agrees that Pet(s) must have had a chance to do their bathroom duties before entering any Mindful Mutts' facilities. This rule sets your Pet(s) and others up for success by not giving them an opportunity to urinate or defecate on the premises and encourages them to relieve themselves at appropriate times when taken out by the staff of Mindful Mutts. Owner understands that group care & boarding are not substitutes for regular leashed walks by Pet's family & that if behavioural issues arise (ie. Eliminating indoors, pulling on leash, leash reactivity) is not the fault of Mindful Mutts. Some dogs may be required to wear diapers/belly bands at the expense of The Owner in order to keep our facility clean.

The Owner understands that their Pet(s) may be asked to leave permanently, no refunds or credits will be given, if we notice any of the following. – Dog – Human aggression – Dog – Dog aggression – Excessive or dangerous guarding – Excessive, stressful barking – Excessive urination or defecation indoors – Med – High Anxiety w/o training protocol acknowledged – Any destruction of property – Owner is not complying with positive training advice and is using aversive tools, or implementing "balanced" training. – Owner is not willing to cooperate by working with a certified, formally educated science-based trainer, referred by Mindful Mutts.
