Please note that this waiver applies to the initial visit and service provided by Mindful Mutts for the client, and a new waiver will be requested to be completed for each animal in the same household , however is binding for all future subsequent visits as well. We reserve the right to adjust, remove or amend all terms reasonably without notice however will note changes in client newsletters.

There are NO refunds for boarding or pet sitting. TRIAL DAYS REQUIRE PAYMENT OF THE GOING RATE LISTED ON OUR WEBSITE JUST AS ANY OTHER BOOKING WOULD REQUIRE. THERE ARE NO REFUNDS OR CREDITS FOR BOARDING INCLUDING TRIALS. PAYMENT IS FOR A RESERVED SPACE, SERVICE IS PROVIDED WHETHER YOUR DOG DOES OR DOES NOT GET APPROVED.

A deposit may be arranged, however, a full payment of bookings is required prior to when a booking begins. You may choose to send an etransfer to info@mindfulmutts.ca with your dogs name in the note section for the subtotal including tax to avoid processing fees .

Once paid, your booking is reserved. There are no refunds, credits may be given on your account minus a $50 administrative fee at the discretion of management. Those credits will expire within 1 year.

All booking are subject to verification by Mindful Mutts and you will be notified if any changes need to be made.

I understand that no new dogs will be accepted over 6 months that have not been spayed/neutered. Dogs that are under 6 months, if accepted, will be required to pause boarding at the discretion of Mindful Mutts at 6 months old due to behavioural challenges that directly become disruptive to our philosophy of "low stress" and "easy management" for the sake of mental & physical health in both YOUR dog, and the others in the group. The Owner understands that Mindful Mutts does not allow vasectomies or ovary-sparing spays in place of spaying or neutering. We may request proof of sterilization, please have it handy just in case!

New Clients must all set up a trial night. This is a 24 hour period in which we will assess if the service and the service providers (whether boarding or pet sitting etc) are appropriate. The cost is non refundable, mandatory, and does not guarantee we can look after your in the future. in some cases we may need dogs to (non exhaustively) be dog friendly, kid friendly, able to rest alone for a couple of hours a day at a time. Someone is likely not going to be with your dog 24/7 when in our care. Separation Anxiety and other challenges may prevent us from helping you with your dog so please consider this and we can discuss it ahead of scheduling a trial day.

Owner agrees to pay the rate of boarding in advance to the date the Pet(s) is scheduled for their stay, or the space will not be guaranteed and will likely be given away. Owner agrees to pay 100% of total cost as a nonrefundable deposit to secure boarding dates required, due as soon as boarding in confirmed in written format.*

Owner further agrees to pay all costs and charges for special services requested, taxi fees and all veterinary costs etc for the Pet(s) during the time said Pet(s) is in the care of Mindful Mutts.*

Owner further agrees that the pet shall not leave the facility until owner pays all (additional) charges to Mindful Mutts. After 3 Months, the facility will take ownership of the animal, and work with local animal services for further instruction.*

By Signing this contract and leaving Pet(s) with Mindful Mutts, the Owner certifies to accuracy of all information about Pet(s) on client information form initially filled out upon becoming a client (which can be found in your online client portal profile).*

Facility shall exercise reasonable care for the Pet(s) delivered by the owner to Mindful Mutts for boarding. The Owner further agrees to be solely responsible for any and all acts of said Pet(s) while in the care of Mindful Mutts.*

We reserve the right to refuse service at any time to protect the integrity, health and safety of our staff, clientele, and community as a whole by being leaders in transparency, health and cleanliness.*I understand

Owner specifically represents to the facility that the pet has received all vaccinations necessary to prevent rabies, parvovirus, distemper, and kennel cough. Owner will provide proof or if the pet is not vaccinated please initial initial.*

Owner specifically represents that he or she is the sole owner of the pet, free and clear of all liens and encumbrances.*

If pet becomes ill or if the state of the animal's health otherwise requires professional attention, Mindful Mutts, in its sole discretion, may engage the services of a veterinarian or administer medicine or other requisite attention to the animal, and the expenses thereof shall be paid by the owner.*

The Owner understands that Mindful Mutts encourages Owners to keep Pet(s) on regular flea and parasite prevention medication regularly. Owner understands if fleas or parasites are found, The Pet(s) will be separated and further action will be taken so as not to spread to other pets. (ie. The Owner will be notified to pick up dog, or Mindful Mutts will take pet to vet for flea bath and treatment etc. Owner will be responsible for payment.) Flea medications are required year round. Owner agrees to upkeep flea treatment and provide proof of most recent treatment if requested.*I understand

In an emergency, Mindful Mutts will try to contact The Owner or emergency contact and personal veterinarian as provided by the Owner. However, the Owner understands and agrees that in an emergency there might not be the time to do so before the administration of care. Therefore, in the event of a sudden illness or injury, The Owner grants Mindful Mutts full authority to make decisions involving medical treatment of the Pet(s) and authorizes Mindful Mutts to take the Pet(s) to the veterinarian at Reimbursement will be required within 14 calendar days of billing, via etransfer to info@mindfulmutts.ca Please specify the amount you authorize Mindful Mutts to spend on The Owner's behalf in case of emergency health requirements, by doing so you agree to these terms:

Owner waives any claim against Mindful Mutts as a result of pet partaking in socialization with other pets. The Owner agrees to waive all liability from Owners of Mindful Mutts, as well as subcontractors, and any other affiliated with Mindful Mutts. Owner agrees to indemnify Mindful Mutts, its successors and assigns, against it as a result of any injuries or diseases inflicted on any other pet at the facility by the Owner's Pet(s).*

I understand that behaviour is not a guarantee and that Mindful Mutts reserves the right to remove a dog that poses a safety issue or does not fall under our definition of "ease of management". Mindful Mutts will not take in dogs that are not suitable for a group environment, safe to have around people as well as dogs, or that begin to develop problematic group behaviour. Some exceptions may be made for dogs working on separation anxiety with the help of professionals ONLY (several restriction apply). Please fill out the "other" section if there is anything we should know ahead of time. *I understand

All dogs must come in on a harness, and a standard 6ft leash WITH ID. ID must be on their harness OR collar (though the collar itself is not a requirement) No shock collars, prong collars, choke chains or Flexi leashes will be permitted.*

If your dog is involved in altercation with another dog while at facility, pet owner will be responsible for all veterinary charges incurred.*

Owner will provide the following during Pet(s) stay at the facility: - Minimum 1 bully stick (or similar) per 48 hours of stay - Chew Toys (if dog is young or major chewer) - Prepared food (ie: Labeled “Breakfast, lunch dinner” with premeasured portions or measuring instructions.) - Clearly labelled medication if any. - Special high value treats with mention of any allergies or sensitivities*

Owner will please not bring any of the following, unless agreed to in writing or email format. - Crate - Rawhide - Dog beds - Personal dishware - Personal utensils - Shock collars - Choke collars - Prong collars*

This is a contract between Mindful Mutts- Group Dog Care + Positive Training Facility and the pet
owner whose signature appears below (hereinafter called “Owner”)
