All dogs must come in on a harness, and a standard 6ft leash WITH ID. ID must be on their harness OR collar (though the collar itself is not a requirement) No shock collars, prong collars, choke chains or Flexi leashes will be permitted.*

This is an Agreement between Mindful Mutts- Positive Training & Consulting, from herein referred to as Mindful Mutts, and the person signing this electronic form, from herein referred to as the Owner. The following are the terms of service for the pet who's name is entered below, from herein referred to as the Pet(s), to stay at Mindful Mutts in any capacity. By filling out this form The Owner certifies that they are eighteen years of age or older and the legal Owner of the Pet(s)*I agree

The Owner acknowledges that this Agreement applies to the current sessions and any subsequent sessions of the Pet(s) at Mindful Mutts. The Owner agrees that appropriate notice will be given in writing by Mindful Mutts if there are any changes or additions to these terms and conditions in order to better serve our clients. The Owner agrees to waive all liability Mindful Mutts, as well as subcontractors and staff hired by them, and any others affiliated with Mindful Mutts*

Once purchased, all private training will be be non refundable, non transferable, and have an expiry date for ALL training credits of 60 days from date of purchase.*
