I am the legal owner/guardian of the dog listed below and affirm my dog is healthy and up to date on vaccinations required for participation in Souly Dog activities.
I acknowledge that participation in dog training, nature therapy, and rewilding activities may expose my dog and myself to risk of injury, illness, or property damage. Risks include but are not limited to dog bites, infectious diseases, contact with other animals or plants, escape, and unpredictable behavior.
I voluntarily assume all risks arising from my participation and my dog’s involvement in Souly Dog services, classes, nature-based programs, outdoor activities, and related events.
I release Souly Dog, its owners, trainers, agents, volunteers, and facility partners from all liability, claims, demands, or causes of action connected with participation, including negligence, except for gross negligence or intentional misconduct.
I agree to defend, indemnify, and hold harmless Souly Dog and associated parties from any injury, property damage, legal claim, or harm resulting from my dog’s actions or health status before, during, or after participation.
I understand that recommendations given by Souly Dog do not guarantee specific behavior outcomes, and my dog’s learning and safety are ultimately my responsibility.
I agree to abide by all rules, guidelines, and instructions provided by Souly Dog staff and trainers.
